How to Report a Claim
Due to SELF's partnership with Sedgewick, SELF utilizes Sedgewick's Global Intake System for claims reporting. There are two options for reporting claims:
Option 1: Global Intake
- Login to portal
- Enter your name and email address (not the injured employee’s name)
- Enter Access Code: 7632SELF (this is not a unique password but should not be widely shared)
- Follow the prompts and enter the requested information
- No unique login name or password is required
Option 2: Email Reporting
- Complete Illinois Form 45: Employer’s First Report of Injury
- Email Form 45 to 7632SELF@sedgwick.com
